一、业务办理温馨提示

1、前来办理业务的出入境人员须通过陕西国际旅行卫生保健中心微信公众号或网站进行实名预约；体检同时并需接种黄热病疫苗的人员，请在“黄热病疫苗接种预约”界面单独预约接种服务。
2、如果不能如约前来办理，请提前一天在“我的预约“取消预约，避免影响下次预约办理
3、健康体检前需保持空腹状态。
4、业务受理时间：工作日8:00-11:00，
5、取证时间：完成体检、接种等业务后3个工作日14:00-17:00。
6、中心只受理成人二类疫苗接种，不受理少儿疫苗接种。
7、我中心无口岸食品、饮用水从业人员、国际交通员工体检资质；无前往加拿大、澳大利亚、新西兰除工作外其他类型健康体检的资质；无移民健康体检资质。
8、PPD皮试项目每周五进行，PPD皮试必须和麻腮风疫苗、水痘疫苗同一天进行或者间隔28天以上，请需要做皮试的学生最好预约周五的体检及接种。

二、中国公民出境体检及预防接种需携带资料：

1、身份证或护照原件；
2、近期4张2寸白底彩色证件照（请勿修照片）4.8 X 3.3cm；

三、中国公民赴境外申请免费体检，请主动出示以下资料复印件，资料作为减免费用依据留存。

1、留学生需提供：签证（1年及以上留学签证）；录取通知书（学制为1年及以上）；I-20表格（时间要求一年及以上，仅留学美国学生提供）。
2、国家留学基金委资助留学生需提供：“国家留学基金资助出国留学资格证书”（资助期限一年及一年以上）。
3、工作提供：一年及以上签证及本次出境1年及以上劳务证明。

四、有其它需要办理的业务，请提前自行准备资料并在前台主动出示申请受理

1、如果本人有需要填写体检或疫苗接种情况的表格：请自行提前打印并提交表格；同时提交填写表格所需的资料。
2、需要办理疫苗接种记录翻译：请携带证件并提交清晰、完整、真实的既往接种记录原件及复印件各一份（仅将中文记录翻译为中英文）；需要翻译新型冠状病毒疫苗接种记录，请携带护照原件（护照复印件或者图片也可），并提供接种记录打印件一份。

Please read the instructions carefully before filling in the form:

1. Please make an appointment in advance, and then you can get the number according to the appointment information. If there is no appointment ,you will not be able to have a physical examination.(Please make sure all information entered by you is true and correct)

2. Please enter your company name/school name/address carefully in Chinese language.

3. The student who want to take the physical examination free of charge is required to submit the copy of following documents:

• Valid Chinese VISA of one year or more (Only for the first entry student).
• School Admission Letter.

4. The foreign expert who want to take the physical examination free of charge is required to submit the copy of following documents:

• Valid Chinese VISA of one year or more (Only for the first entry expert).

5. If you have completed the entire physical examination in your own country .Please read the following instructions carefully:

• Physical examination results must be provided by domestic public hospitals ,Medical examination results issued by private clinics are unacceptable.
• Physical examination must be done within 6 months before arrival in China.
• Please attach all the original laboratory results.
• Please bring along chest X-ray film（Chest X-ray must be done within 6 months.）

• We reserves the right to repeat full medical check up or any specific laboratory test should there be any doubt in the medical report submitted.

黄热病疫苗接种请提前在陕西国际旅行卫生保健中心微信公众号或官网预约，请务必按照预约时间前来办理。

黄热病疫苗只为前往世卫组织所公布的黄热病风险的国家或地区人员接种（后附具体国家或地区名录）。

黄热病疫苗接种后10天产生保护效力，请合理安排时间。

接种需携带的资料：

1、黄热病疫苗接种须凭前往世卫组织所公布的黄热病风险的国家或地区签证复印件或机票行程单复印件；海员凭海员证复印件办理。
